2023-2024 Athletic Participation Information and Forms

The current school year and sports season will conclude at the end of June 2023.  This means that your physical from a medical doctor must be updated for the 2023-2024 school year and uploaded to Final Forms.

Physicals are good for 395 days or 13 months.  Fall sports begin on August 1.  For you to participate on August 1, you must have a CURRENT physical on file.  

In addition to a physical, every athlete must complete a “packet” from Davidson County Schools that includes Gfeller-Waller forms, Insurance information, Residency verification, and other information which requires both parents and student to sign. These forms can be found on Final Forms and must be submitted through Final Forms. Athletes only have to fill this out ONCE per school year, not for every sport they play.  The current physical form is available for download on the right side of this page.  Follow “LEDFORD PROUD” on social media for news, links, and information.
